  • Annual deductible $25 individual / $75 family
  • Covers:
    • 100% of Class I Preventive Dental Services (deductible does not apply)
      Examples: exams, cleanings, x-rays
    • 90% of Class II Basic Dental Services
      Examples: fillings, extractions, restorative treatment, repairs to crowns or bridges, non-surgical periodontics
    • 50% of Class III Major Dental Services
      Examples: oral surgery, endodontics, inlays, onlays, crowns, prosthetics
  • 6-month waiting period applies for Class III benefits
  • $1,500 annual maximum per individual
  • Orthodontia not covered
  • Use any dentist but participating network dentists have agreed not to bill more than the insurance company’s Maximum Allowable Charge
