You may go to any doctor or hospital for services, but, if you choose a doctor or hospital in the PHCS PPO network, the charge may be lower. You can find a participating doctor or hospital by visiting the PHCS website – www.multiplan.com – or by calling toll-free 1-800-922-4362.
There are no penalties for not choosing a PHCS provider. Whether or not you use a participating doctor or hospital, the insurance benefits of the plan are the same.
Please note that this insurance plan may not cover the entire bill. You will be responsible for any balance.
Information about locating a doctor or hospital in the the PHCS PPO Network will also be provided on your ID card.
