Custom quotes can be requested when our “standardized” plan designs and rates just don’t fit for a particular group. We ask that you provide the following pieces of information to help us underwrite the case and design plans and rates that are appropriate to the group’s dynamics: - group name and address;
- description of class(es) of eligible employees;
- level of employer contribution;
- electronic census data including employees’ date of birth, gender and state of residence (if electronic data is not available, paper data with summary information is acceptable);
- sample plan designs requested;
- target pricing and/or information on employees’ wages and hours worked per week;
- premium and claims experience information for the preceding three years, if available;
- plan design and rate information on any in force coverage and why the group is seeking new coverage.
