This plan is available to any non-discriminatory class or classes of full-time, part-time, seasonal or temporary employees designated by the employer. Coverage may also be made available to insured employees’ families at the employer’s option.
Enrollment Requirements:
- Employee must be actively-at-work on the effective date of insurance or the next work day following the effective date of insurance
- There are no health questions or physical exams required to qualify
- There are no minimum participation requirements
Eligible employees may enroll:
- within 30 days of the date employment begins or upon completion of any required waiting period
- after a HIPAA life-status event occurs, or
- during any annual open enrollment period.
Coverage is effective the first day of the month following the date the enrollment form is processed provided the employee and/or dependent(s) are actively at work on that day or on the next business day if the first of the month falls on a weekend or a holiday.
For dependents who are not employed, “actively at work” means that they are able to engage in substantially all of the usual activities of a person in good health who is the same age and sex and are not confined in a hospital or other facility.
Note: A six month pre-existing condition limitation applies but only to hospital & surgery benefits.
If the Optional Dental Plan is elected, some services require a 12 month waiting period before benefits are available.
